We look forward to working with you and your family. Our goal is to maximize your functioning and to reduce suffering. For this process to be as successful as possible we will need your help to explore all of the possible causes of your ongoing symptoms.

Initial Appointment

Your initial appointment may be 2 hours in duration: 1 1/2 hours will be for the evaluation and 30 minutes will be for education and feedback. Finally, we may discuss research projects with you and your family to provide you an opportunity to help us improve our knowledge and care of children and young adults with chronic pain. Please plan to arrive at the center 20 minutes prior to your scheduled appointment time to register and complete any necessary paperwork. It is very important that you arrive on time to keep your appointment for this date. If you arrive late, you may be asked to reschedule your appointment. We appreciate having as many family members present at the initial evaluation as is reasonable. Please bring all medication bottles that you are currently taking.


We will make every effort to maintain our schedule and yours. If you are unable to keep your appointment we ask that you give us at least 48 hours notice with a telephone call to reschedule. If you cancel/reschedule with less than 48 hours notice you may be held financially responsible for the appointment.


Since all insurance company policies are different, it is advisable that you become familiar with your particular insurance coverage. This allows us to assist you in obtaining your maximum benefits. We will be collecting your co-payments at the time of registration. Many people have insurance policies that aid in the cost of their child’s medical care. Please remember that insurance is meant to reimburse the patient for fees paid to the doctor and is not a substitute for payment. We are providers for a number of insurance companies in which case we can bill your insurance directly for any charges above the co-pay amount or deductible.

It is your responsibility to pay any deductible amount, co-pay, or any balance not paid by your insurance company at the time of the visit. If we are not a provider on your insurance plan we still bill your insurance as a courtesy. However, you are responsible for verification of coverage and any remaining balance. We understand that finances can be a sensitive issue but we encourage you to contact us early regarding any problems.
